Module Description:  According to the Journal of Community Hospitals (2016) preventable medication errors impact more than 7 million patients and cost almost $21 billion annually across all care settings. About 30% of hospitalized patients have at least one discrepancy on discharge medication reconciliation.  A person-centered approach to medication safety results in reduced medication related problems, increased quality of care and overall reduction in prescription cascades.

Learning Objectives

  • Identify strategies to prevent medication-related problems in older adults.
  • Define the role of healthcare professionals and team members as partners in resolving medication-related problems and improving medication safety.
  • Apply the strategies for preventing medication-related problems on Case Studies
